Since you have already passed the Fachsprachprüfung (FSP) and are an EU citizen who studied outside the EU, you basically have two options for starting your career in Germany: the Hospitation or the Berufserlaubnis.
A Hospitation is a purely observational activity in a clinic. You participate in the daily routine of the clinic, get to know the procedures, but are not allowed to take on any medical tasks. For newly graduated doctors, this is often a good first step to get to know the German healthcare system. However, it is usually unpaid or only associated with a small expense allowance. You do not need to apply for a Hospitation – you can arrange it directly with a clinic, as long as you do not perform any medical activities. The Berufserlaubnis, on the other hand, is a temporary work permit with which you can work clinically as an Assistenzarzt under supervision. It is usually granted for up to two years. With it, you can already gain practical experience, earn a regular salary and at the same time prepare for the Kenntnisprüfung, which later leads to the Approbation. The prerequisite is the passed FSP – which you have already fulfilled.
So if you are fresh out of university and want to gain practical experience right away, I recommend that you apply for the Berufserlaubnis. This is the most effective way to develop professionally and prepare for the Approbation. A Hospitation can be useful if you are still unsure whether a particular clinic is right for you or if you want to gain insights without responsibility at first. However, it does not replace medical activity.
