No – you don't have to be a specialist doctor yet to start as a doctor in Germany. The typical path is:
1) Approbation (full medical license) OR in some cases a temporary Berufserlaubnis first
2) Entry as an Assistenzarzt (medical activity within the framework of Weiterbildung)
3) Facharzt = completion of Weiterbildung after several years (depending on the specialty)
