• Use a fixed skeleton every time: current complaint (OPQRST), past medical history, medications, allergies, family, social, then a quick systems review. Knowing the order stops you from wandering.
• Steer the patient politely: “Darf ich kurz zusammenfassen?” and “Ich komme gleich darauf zurück” let you take back control without being rude.
• Time-box it in practice: set a timer and force yourself to close the history at a fixed minute, even if imperfect. Then practice the handover and Arztbrief with the remaining time.
• Don’t aim for every detail — aim for the safety-relevant ones (red flags, meds, allergies).
