Is the FSP noticeably harder in some Bundesländer than others?

I can register in Hamburg or in Niedersachsen. People say the FSP is harder or easier depending on the chamber. Is that true or just rumours?

There are small differences in organisation and examiner style between chambers, but the core standard is the same everywhere: it is a C1-level medical communication exam with the same components (history taking, doctor-patient explanation, Arztbrief, doctor-to-doctor handover). A few realistic points:
• “Easier/harder” reputations are mostly anecdotes — they often reflect one person’s examiner or a bad day, not a systematic gap.
• Practical factors matter more: waiting times for a slot, location/travel, and where you actually live and want to work (this ties into BE and visa).
• Don’t pick a chamber hoping for an “easy” exam. Pick based on logistics and slot availability, then prepare to the C1 medical standard.
If you prepare properly, you’ll pass in either Hamburg or Niedersachsen.

its mostly rumours imo. the format is similar everywhere (anamnese, arztbrief, doctor-doctor talk). examiners differ a bit but the standard is the same C1 medical level

i took it in two different states (failed once, moved once) and honestly the level felt the same. the examiners were just different personalities

more important than difficulty is the WAITING time for a slot. one chamber made me wait 2 months, another had space in 3 weeks. pick by availability
