Recognition for doctors: Approbation, Berufserlaubnis and the exams
Doctors need Approbation to practise independently. A limited Berufserlaubnis may allow supervised work while an application is pending.
Eligible EU, EEA or Swiss qualifications can receive automatic recognition, but you must still apply and meet the other licensing conditions. For other qualifications, the state checks equivalence; substantial differences may require a Kenntnisprüfung.
The state authority decides the licence. It usually asks for B2 general German and a C1-level medical Fachsprachprüfung; ask about accepted evidence and any exemption.
Change scheduled for November 2026
A published 2026 reform makes the direct Kenntnisprüfung the usual route for third-country medical qualifications from 1 November 2026, while retaining a document-based option. Ask the authority which rules apply to your filing.
Documents and German level for the Approbation
Use the document list issued by the authority in the state where you plan to work. Common evidence includes:
- Degree, transcript and curriculum with training hours; evidence of clinical placements and professional experience.
- Identity document, evidence of professional standing, and proof of personal and medical fitness. Check each document’s validity period.
- German-language evidence: usually B2 general German and a C1-level medical Fachsprachprüfung. The authority tells you when to provide it.
- Translations and certified copies in the form requested by that authority. Ask before obtaining an apostille or sending any original.
How long the Approbation takes and what it costs
The authority confirms receipt and missing documents, then assesses a complete file. EU automatic recognition can be quicker; equivalence assessment, exam dates and missing papers extend the overall route.
Budget separately for authority fees, translations and any language or knowledge exams. Prices and dates vary by state and provider.
- Knowledge exam format
- Oral-practical; usually 60–90 minutes.
- Knowledge exam attempts
- Initial sitting plus two retakes.
- Berufserlaubnis
- Normally up to two years; exceptions possible.
Which state office decides your Approbation
Apply to the competent authority for the state where you intend to work. A medical chamber often administers the Fachsprachprüfung; the authority decides Approbation.
| Federal state | Approbation authority |

| North Rhine-Westphalia | Bezirksregierung MünsterMünster |
| Bavaria | Regierung von OberbayernMünchen |
| Baden-Württemberg | Regierungspräsidium Stuttgart - Referat 98 - Landesanerkennungsstelle für GesundheitsberufeStuttgart |
Approbation applies nationwide. A Berufserlaubnis may restrict location, employer or duties; read the permit before changing jobs.
Working as a doctor while you wait
Before Approbation, practise medicine only within the terms of a granted Berufserlaubnis. A job offer and supervision may be required.
A Hospitation is observation, not permission to treat patients. If you plan to enter through an Anerkennungspartnerschaft, check its visa, employer and recognition conditions separately.
