Anerkennung as Notfallsanitäter or Rettungssanitäter: two titles, two laws
Notfallsanitäter is a protected federal title with a three-year training programme. The state authority compares foreign training and experience with that reference profession and checks health, reliability and German-language requirements before granting title authorisation.
Rettungssanitäter is organised under state law. Its training and any recognition procedure are separate from the Notfallsanitäter licence. A foreign Notfallsanitäter application does not automatically produce Rettungssanitäter recognition; ask the target state whether and how it assesses the second title.
Documents, German level and the driving licence
For a Notfallsanitäter application, the competent office may request:
- Your diploma and detailed records of classroom, ambulance and clinical training.
- Documented emergency-care experience and other qualifications relevant to the comparison.
- Identity documents and translations or certified copies specified by the office.
- Health, reliability and German-language evidence for the title authorisation.
- If you will drive, ask for the ambulance’s permitted maximum mass: category B covers up to 3.5 tonnes; a vehicle above 3.5 and up to 7.5 tonnes generally needs C1. Verify foreign-licence validity separately.
How long it takes and what drives the cost
For Notfallsanitäter, the federal training regulation sets a decision period of up to four months once the required documents are complete, with special rules for accelerated procedures. A notice requiring an adaptation period or test adds further time.
Rettungssanitäter deadlines and fees depend on the state procedure, if one exists. Ask the authority and employer before budgeting for translations, a compensation measure or driving-licence conversion.

Working while you wait: Rettungssanitäter level and the adaptation course
Emergency response roles have different duties and title requirements. Before accepting work, ask the employer which qualification the position requires and whether your current authorisation covers its tasks.
Applicants from outside the EU also need a residence and employment route that permits the proposed work or qualification measure. Recognition, employment and driving permission are separate checks.
